3. Indications for Use.

Dexamethasone should be used for treatment of shock in process of after a trauma or surgery, circulatory collapse (severe lowering of arterial tension), loss of blood, burns, myocardial infarction. Can be applied in severe infections: vascular collapse (severe lowering of arterial tension), toxemia (presence of toxins substances that may cause disease or death), meningococcosis (purulent inflammation of linings of brain - meningitis), diphtheria, septic infection (a form of infection of blood caused by microorganisms), abdominal typhoid, grippe, pneumonia (inflammation of lungs), peritonitis (peritoneal inflammation), eclampsia (toxicosis of the second part of pregnancy). Acute reactions of allergies: laryngeal edema, asthmatic status (long-running bronchial asthma attack, resistant to treatment with usually applied medications), dermatosis (skin disease), serum transfer, acute anaphylaxis reaction (immediate allergic reaction) to different drugs, including antibiotics, pyrogenic reactions (elevation of the temperature of body).

4. Action

The medication is an adrenal hormone, which has the ability to influence the metabolism of carbohydrates and protein, as well as shows anti-inflammatory and anti-allergic effect. The medication slows down migration of leucocytes to the zone of inflammation.

5. Posology and Administration

At the start of treatment and when using the medication for treatment of acute form of illness, the medication should be used in higher doses. After achieving the necessary effect of treatment, it is necessary to decrease the dose as well as interval between doses to several days, until reaching the level of supporting treatment or up to cancellation of treatment. The doses should be chosen individually. When the medication is used for treatment of severe conditions at the start of treatment it should be used in dose of 10-15mg per day, and the supporting dose should be from 2 to 4.5mg per day or more. For treatment of acute allergic conditions and asthmatic conditions the medication should be used in dose of 2-3mg per day, which should be taken in a small period of time. When the medication is used for treatment of adrenogenital syndrome (compromised adrenal cortex functioning, combined with supersecretion of sex hormones in males) the dose should be chosen individually, depending on the excretion on 17-ketosteroids with urine. The effect of treatment is usually achieved after intake of 1-1.5mg of medication. The average dose per day is 2-3mg per day, which should be split in 2-3 intakes. When the medication is used for treatment in small doses, then it is necessary to use the medication during morning time.

6. Counterindications

Counterindicated for oral and parenteral and systematic use in patients with III and IV level of obesity and Itsenko and Cushing syndrome, with convulsive states, open-angle glaucoma, hypoalbuminemia and in conditions predisposing to its emergence.

For introduction intra-articularly: fugacity or non-efficiency of the previous two intakes, systemic grave condition of the patient.

7. Should Be Taken with Precautions, if…

It is necessary to survey a patient before starting a treatment in order to understand the possible counterindications.
The medication should be administered with caution for treatment of patients, who have nonspecific ulcerative colitis (presence of blood in stool), as well as presence of fecal concretions, kidney failures and duodenal or gastric ulcers, systemic osteoporosis, arterial hypotension, trembling.
Dexamethasone should also be administered with caution to patients, who suffer from diabetes, evidenced arterial hypertension, mental diseases. In case of having liver cirrhosis, as well as compromised function of thyroid gland Dexamethasone may have enhanced performance. IN case of having basic emotional instability or psychotic tendencies, Dexamethasone can enhance the effects.
When Dexamethasone is used for treatment of patients with herpes corneae, it is necessary to understand that there is possibility of perforation of cornea.
When Dexamethasone is withdrawn abruptly, particularly when previously used in high doses, the medication can cause cancellation syndrome.

8. Interface with Other Medications

Treatment with Dexamethasone or its combination with any other medications is only allowed according to order of a doctor. Inform your doctor if you are taking any medications.

9. Overdosing

Overdosing. Symptoms of getting overdosed: enhancement of side effects, described below. When the dose of Dexamethasone used for treatment is reduced, the overdosage symptoms go away, or it is possible to withdraw treatment, if needed.

Vomiting, nausea, bradycardia, heart arrest, arrhythmias, delay of natrium, aggravation of heart failure, edemas, hypokalemia, muscle weakness, high arterial tension; reduction of muscle volume, compression of spinal bones fractures, reduction of bone tissue, pathologic fractures of tubular bones, steroid ulcers of intestinal tract, non-inflammatory impairment of cardiac muscle; bleeding from ulcers, pancreatitis )[articularly in patients, who abuse alcohol), perforation of intestinal wall, increase of the liver; thinning of the skin, delay in wound healing, sweatiness, rash, dermahemia of the face, hematomas, spots, pimples, blue strips or distensions on the skin; convulsions, epilepsy; elevated pilosis of facial skin and skin of arms, back, parasecretion of sex hormones (disorders of menstrual cycle, impotency); impairment of growth in children, Itsenko-Kushing syndrome, progression of probability of diabetes; increased body mass, high intraocular tension; bulimia, immunodepression, elevated fatigability, change of mood, high intracranial tension, mental affections, insomnia, dizziness, mental disorders; headache, more often development of infections and aggravation of their course, adrenal insufficiency and atrophy of adrenal cortex, especially during stress (in case of having surgery, trauma, coexistent diseases); rare – allergic reactions (itching, rash).

11. Pregnancy and Lactation

Dexamethasone is only allowed for use during the first trimester of pregnancy and during the period lactation in case of life-saving conditions and taking into account the fact that expected positive outcome for mother is more important than negative effect on fetus. Treatment with Dexamethasone can cause serious impairment of fetus development.

12. Driving

Dexamethasone treatment should not be performed during work of people, whose professional activity needs alertness and concentration of attention.
