  1. Indications for Use.

Is used for treatment and individual prevention of different kinds of extraenteric amebiasis and malaria. Can be used for treatment of subacute and chronic forms of dermatoses, systemic lupus erythematosus, rheumatoid arthritis. Is applied for curing of some forms of abnormal heart rhythms.

  1. Action

Chingamine (Chloroquine) is a medication, which has rapid action on activators of malaria. It has the ability to remove disturbance of rhythm. When it is used for treatment of rheumatoid arthritis, its effect is seen in gradual pain relieving, reducing stuffiness and improving mobility of joints. The medication has swift effect and is well-absorbed and then slowly evolved out from the body.

  1. Posology and Administration

Dose for treatment should be set individually, basing on the age of patient and indications. The medication should be applied according to special schemes.
For prevention of malaria the medication should be taken orally in the dose of 0.5g for a period of 2 weeks, and then once a week on the same day.
When using Chingamine for treatment of malaria in adult patient the medication should be taken orally in the dose of 2-2.5g after a meal during the course of treatment. The first intake of medication should be 1g ( 4 0/25mg tablets), after 6-8 hours it is necessary to take another 0.5g, and on the second and third day you will have to take 0.5g in one intake. Chingamine should be administered in additional 0.5g on the first day of therapy, especially, if treating tropical malaria. For treatment of tropical malaria the treatment can be as long as 5 days, with 0.5g administered on fourth and fifth day of treatment.
The medication is usually used for oral intake, but in some cases of malaria in malignant courses of treatment, it can start with intramuscular administration. Single-shot dose for adult patients in this case is 10 ml of 5% solution. The injections can be performed as often as once in 6-8 hours and up to three times a day. The maximal daily dose per day is 30ml.
The medication can also be used intravenously, but only in extremely severe cases. When using it intravenously, dissolve 10ml of 5% solution in 10-20ml of glucose solution (40%) of isotonic solution of sodium chloride. It is recommended to perform intravenous introduction slowly.
As soon as patient’s condition has been visibly improved, it is recommended to transfer to oral intake.
When the medication is used for treatment of rheumatoid arthritis, it should be taken in the dose of 1 tablet (0.25g) one time a day after having dinner and 2-3 hours before going to sleep. The treatment should be from 3 to 6 weeks but in some cases can be as long as 3-6 months.
When the medication is used for treatment of lupus erythematosus is subacute courses, it should be used for a period of 10 days in the dose of 0.25, which should be taken twice a day after main meals (lunch and dinner), and in future treatment – one time a day in dose of 0.25g after dinner. The complete course of treatment includes from 17.5 to 25g (70-100 tablets). For treatment of acute systematic cases of lupus erythematosus the medication should be taken in combination with hormonal medications. In this case for the first several days of treatment Chingamine should be taken in dose of 0.25g once a day before going to bed. In case of high-tolerability, it can be taken in doses of 2 tablets (0.25g each) during the day. The entire course of treatment has to be from 25 to 30g (or 100-120 tablets). During spring time, when the treatment is aiming at lowering bodily hypersensitivity to sunlight, Chingamine should be taken preventively – for the first intake use 1 tablet of 0.25g one time a day, and then use 2-3 tablets per week.
In some cases Chingamine can be used as arrhythmic medication, aimed at removing failures of rate of heart. In this case it should be taken in dose of 0.25g 2-3 times per day. It is recommended to gradually lower the dose for treatment, until reaching the level of 0.25g per day. For intravenous introduction in this case the medication should be used in dose of 0.5g for cutting of short arrhythmia (repeated introduction of 0.25g).
The highest doses for treatment of adult patients for oral introduction in single-shot is 0,5 g, and 1.5g per day.

The medication can be used for treatment of children and the dose should be defined depending on the age group. For the first day of treatment children of the age group under 1 year old should take 0.05g, age 1 to 6 – 0.0125g, age 6 to 10 years – 0.25g, age 10 to 15 years – 0.5g. For the second and the third day of treatment the medication should be taken in doses 0,025; 0,05; 0,0125; 0,25g respectively.
When the medication is used for treatment of elderly people, it should be used by the same schemes, as for treatment of children of young age or adolescents.

  1. Counterindications

Disorder of functioning of kidneys or liver, hypersensitivity, severe impairments of cardiac muscle (with disorders of rhythm), diseases of cornea and retina of eye, illnesses of nervous system and blood, chronic alcoholism, diabetes.

When Chingamine is used for treatment, it is required to perform regular urine analysis and clinical control of blood, as well as control functioning of liver and in some cases check the condition of eyes.

  1. Interface with Other Medications

  1. Overdosing

Symptoms of getting overdosed: impairment of consciousness, headache, vomiting, convulsions, disturbance of vision, and coma. Possibility of fatal outcome, caused by respiratory distress (possible during 2 hours after the intake).

When Chingamine is used orally in therapeutic doses for a short term, it is usually tolerated well and without evidenced side effects. In case of long-term administration, there is possibility of having dermatitis (inflammation of skin). In case of developing this side effects, it is necessary to lower the dose, and if the symptoms don’t disappear – cancel use of medication. Other side effects include headache, dizziness, nausea, noise in ears, in some cases vomiting, visual distortion. Such symptoms usually go away by themselves. Lowering of appetite is possible during treatment, as well as stomach pains, caused by irritation of stomach mucous coat. Soma patients might have temporary reduction of body mass.

  1. Pregnancy and Lactation

It is only allowed to use Chingamine during pregnancy in case of serious necessity, which should be defined by a doctor. If it is needed to use Chingamine during lactation period, it should be used only if breast-feeding is stopped.
